We make every effort to keep down the cost of your surgical care. An estimate of the charge for each procedure and/or surgery you may require will be given to you upon request. If you have any dental and/or medical insurance as a courtesy to you we will submit claims on your behalf. Payment is required and due at the time of service.
Any co-pays, co-insurance, deductibles, non-covered procedures or amount over the maximum allowable benefits will be due at the time of service. Verification of your insurance benefits is completed as a courtesy to you. Your insurance policy is a contract between you and your insurance company, and therefore we are not able to guarantee benefits or payments that will be made by them on your behalf. All balances deemed patient responsibility are non-negotiable. You will be responsible for all amounts that your insurance company does not reimburse for including but not limited to non-covered services, services they deem not medically necessary, services that are over the insurance companies customary fee schedule or payments received that are less than the estimated amount. If coverage is not in effect at the time of service the OFSA fee will be charged. If you would like our office to submit a pre-determination to your insurance company prior to services please inform our treatment coordinator.
Please remember that insurance is considered a method of reimbursing the patient the fees paid to the doctor and is not a substitute for payment.
