Here’s a day-by-day guide to help you know exactly what to expect in your first week after surgery.
Day 1: Surgery Day
Your mouth will still be numb for the first few hours after surgery, so don’t be surprised if you can’t fully feel your lips, tongue, or cheeks right away.
You might experience some off-and-on bleeding, which is completely normal. We’ll send you home with clear instructions to manage it.
Make sure to start your medications as prescribed — usually alternating between ibuprofen and a stronger pain reliever like tramadol. This keeps you ahead of any soreness once the numbness wears off.
You can eat at any point, but start with something cold and soft (think ice cream, yogurt, or a smoothie without a straw). Swelling typically hasn’t kicked in yet, but it’s important to take it easy: no heavy lifting, running, or anything strenuous.

Day 2: The Soreness Begins
By now, the numbness has worn off — and the soreness is setting in.
Stay consistent with your medications and avoid strenuous activities.
Depending on how complicated your extractions were, some patients feel well enough to go back to work or school on Day 2. If you’re feeling up to it, just listen to your body (and your surgeon’s instructions).
✅ Pro Tip: Keep soft foods on hand like mashed potatoes, soup, or scrambled eggs.
Day 3: Swelling Peaks
Day 3 is when things often feel a little more intense.
Swelling and soreness are usually at their highest point around now, and you might notice your mouth doesn’t open as wide as usual.
The good news? You can slowly start introducing more foods into your diet — mac and cheese, sandwiches, and hot dogs are all back on the table (literally).
You can also begin light exercise again if you’re feeling up for it, but ease into it.
✅ Pro Tip: Gentle movement is good — just avoid anything that increases pressure in your mouth, like heavy weightlifting or intense cardio.
Day 4: Peak Soreness + Watch for Dry Socket
Day 4 can be a tough one — it’s often the peak for swelling and soreness.
If something doesn’t feel right, keep an eye out for signs of dry socket. It’s rare (only about 5% of patients get it), but if you have sharp, intense pain radiating to your ear that isn’t relieved by medication, give us a call.
You might also notice bad breath or a bad taste in your mouth starting today. That’s normal and will start to improve by Day 7.
✅ Pro Tip: Stick to soft foods and gently rinse with the prescribed mouthwash or warm salt water (as directed).
Days 5–6: Turning the Corner
Good news — by Days 5 and 6, you should be feeling much better!
Most patients can start weaning off their pain medications by now or even stop them completely.
Swelling and soreness are noticeably reduced, and your energy should start coming back.
Your jaw might still feel a little stiff, but it’s all part of the healing process.

Day 7: Back to (Almost) Normal
You made it through the first week! ????
By Day 7, you can begin gently irrigating the sockets to help get rid of lingering bad breath or taste. (We’ll show you exactly how to do this.)
You can also start using straws again — but sip carefully.
Your diet should be pretty much back to normal, and most patients feel ready to fully return to their regular activities.
